U.S. Highway 1, north of Ronald Road

Vehicle-1 and Non-motorist-1 (bicyclist) were traveling north on U.S. Highway 1, north of Ronald Road. Vehicle-1 was approaching Non-motorist-1 from the rear. Non-motorist-1 was traveling north on the paved apron. Vehicle-1 veered onto the paved apron and struck Non-motorist-1. The impact caused Non-motorist-1 to tumble onto the east shoulder and into a ditch filled with water. Vehicle-1 fled the scene. I-95 NB/Mile Marker 309

A white Ford E-250 (V01) was disabled on the right shoulder of Interstate 95 northbound. A vehicle traveling (V02) northbound collided with the drivers side, then collided with a pedestrian near the front fender of V01. V02 left the scene. The possible suspect vehicle is a semi truck or motorhome white in color that has a broken right headlight and possible dark gray transfer paint on the bumper. The crash would have occurred between 11/21/14 at 11:30 P.M. and 11/22/14 at 6:20 A.M. If anyone has information pertaining to this case please contact Cpl. CR 220

The Florida Highway Patrol is seeking any information about the driver and/or vehicle involved in a fatal hit and run collision that occurred on October 17, 2014 around 11:22 p.m. on CR 220. The collision occurred directly in front of Mercury Moon Bar/Grill and Whitey's Fish Camp in the Fleming Island area. The suspect vehicle pulled out in front of a motorcycle causing it to strike the vehicle in the driver's side area. The suspect vehicle is described as a dark colored compact sedan such as Toyota Camry, Toyota Corolla, Nissan Sentra, or similar vehicle. There should be damage to this vehicle in the driver side door area and a broken driver's window. The vehicle may have also had tinted windows. The Florida Highway Patrol is interested in any additional information on this case. Sergeant D. Smith and Corporal A. Bennett are the lead investigators for this investigation (FHPG14OFF043394). Hibiscus Avenue

The Florida Highway Patrol is attempting to obtain information on a person or vehicle involved in a hit and run traffic crash. The traffic crash occurred on August 31, 2014 during the morning hours upon Hibiscus Avenue in Middleburg, Florida. An unknown type of vehicle struck a pedestrian while walking along the roadway. The pedestrian is currently in critical condition and receiving medical treatment at UF Health of Jacksonville. This vehicle shall have damage to the right front portion near the headlight assembly. In addition, the vehicle may be described as a year 2010 - 2014 domestic vehicle (GMC, Chevrolet, Dodge, Ford, and some Volkswagen). The driver then left the scene to an unknown destination. Our investigators are interested in any information on the persons and/or vehicle involved in this case and encourages the public's assistance. Beach Boulevard and Carmichael

The driver of Vehicl 1 was operating a gocart in the westbound inside traffic lane of Beach Boulevard at the intersection of Carmichael without any lights displaying on the gocart. The driver of Vehicle 2 was traveling westbound in the inside traffic lane of Beach Boulevard approaching Carmichael. The driver of Vehicle 2 applied brakes and struck the gocart. After the collision, the driver of Vehicle 2 fled the area. Vehicle parts were left at the area of collision. At this time it is believed Vehicle 2 is a pick-up truck or sport utility vehicle with undercarriage damage and headlight damage. It is unknown at this time which headlight is damaged. I95 Northbound Rest Area/Mile Marker 331

A female pedestrian was struck by a vehicle in the northbound rest area on I-95 at the 331 mile marker. The crash took place in the tractor trailer parking area at the rest area. There were no witnesses to the crash and no vehicle description. US Highway 17 NB south of Reed Bell Street

A white Honda Accord was traveling northbound on US 17 south of Reed-Bell Street. The Honda struck a 3 year old child who was in the roadway. The vehicle continued northbound on US 17. The child was pronounced deceased at the scene. The vehicle was later located by Putnam County Sheriff’s Office deputies. The investigation is ongoing as to why the child was in the roadway and the identity of the driver of the hit and run vehicle. Interstate 95 at West 8th Street

For unknown reasons at this time, Vehicle-One (V-1) was stopped northbound on Interstate 95 at West 8th Street in the center travel lane. Vehicle-Two (V-2) was traveling northbound on Interstate 95 approaching West 8th Street in the center travel lane, directly behind V-1. According to the driver of V-2, he was approaching West 8th Street in the center travel lane and did not observe V-1 until the last minute due to the rear lights of V-1 not illuminating and the street light in the area of the crash being out. Consequently, the driver of V-2 was unable to slow in time and as he began to apply his brakes V-2 began to skid. The front of V-2 collided with the rear of V-1. Both V-1 and V-2 were redirected onto the east paved shoulder and the front of each vehicle collided with the metal guardrail on the east shoulder of Interstate 95. Subsequently, the driver of V-1 was ejected from V-1 and was lying unconscious in the center travel lane. An independent witness stated the driver of V-1 was then struck by two unknown vehicles, which continued northbound on Interstate 95. US 1 South of M. Sikes Road

Pedestrian-1 was a passenger in a vehicle that was involved in a crash that occurred on U S 1 south of M. Sikes Road. P-1 exited the crash vehicle on the passenger side and walked around the crashed vehicle to the left front. V-1, a long nose semi truck tractor, was also traveling southbound on U S 1 south of M. Sikes Road. The driver of V-1 took evasive action to the left to avoid the crashed vehicle in the roadway. The right front of V-1 struck P-1 while she was standing in the roadway. The driver of V-1 stopped in the outside travel lane south of the crash. Driver-1 exited and walked back to the crash site. A short time later D-1 got back in his truck tractor and left the scene. V-1 was described as a long nosed, orange colored tractor pulling a white box trailer. St. Johns Bluff Road near Lost Pine Blvd

The Florida Highway Patrol is currently looking for a pickup or sport utility vehicle, possibly white or silver colored, which was traveling northbound on St. Johns Bluff Road near Lost Pine Blvd. The suspect vehicle struck a male pedestrian with its right front. The suspect vehicle did not stop and fled in an unknown direction. Interstate 295 at Pritchard Road

Vehicle-1 and Vehicle-2 were traveling northbound on Interstate 295 at Pritchard Road. V-1 was traveling behind V-2. The right front passenger of V-2 started throwing trash out of the passenger side window at the windshield of V-1. This happened three different times. The driver of V-1 changed lanes to the right lane to go around V-2. The driver of V-2 steered his vehicle to the right and forced V-1 off the roadway. The driver of V-1 lost control and overturned. The rear seat passenger was ejected from V-1 and died a few days later. V-2 left the scene. The only information for V-2 was a mid-size red pickup truck. The windows had dark tinted windows with a "Salt Life" sticker on the back window and a Florida plate with a frame around it with Georgia Bulldog insignia on it.

State Road 16

Vehicle-1 was traveling eastbound in the outside lane on State Road 16. Pedestrian-1 was traveling eastbound in the designated bike path on State Road 16. V-1 attempted a lane change into the right turn lane in preparation for a right turn onto Four Mile Road. V-1 front struck P-1. P-1 was propelled northeast. V-1 left the scene. P-1 died within 24 hours.

Interstate 95 northbound and Baymeadows Road

The Florida Highway Patrol is currently looking for an unknown vehicle which was traveling northbound in right lane of a construction area on i-95 northbound and Baymeadows Road. The pedestrian was struck and killed with right side of vehicle. The suspect vehicle may have lost its right side mirror in the collision.

Westbound J. Turner Butler Blvd. @ Kernan Blvd.

The Florida Highway Patrol is currently looking for a vehicle that was traveling on JTB eastbound at a high rate of speed. The suspect vehicle then rear ended another vehicle causing the victim to strike the guardrail. The suspect vehicle fled the scene. The suspect vehicle was described as a Volkswagen Beetle. The suspect vehicle was later located by a security guard at apartment complex using CrimeStoppers Hotline.

Timuquana Road

Pedestrian-1 was walking eastbound on the sidewalk on the south shoulder of Timuquana Road. According to a witness, P-1 was staggering as he walked and appeared to be intoxicated. Vehicle-1 was traveling eastbound on Timuquana Road in the outside traffic lane. At some point P-1 staggered into the outside traffic lane in the path of V-1. P-1 was struck by the front of V-1. V-1 traveled approximately 200 feet east of the crash scene and stopped. The driver and passenger of V-1 exchanged places and left the scene. P-1 was pronounced deceased at the scene by rescue personnel. No tag information available on V-1. Possible description of V-1 by witnesses was a white van with a blue strip, possibly a Chevrolet Astro Van.

I-95 Northbound and Old St. Augustine Road

A pedestrian left the emergency department of Baptist Medical Center – South Campus (out the back door) and attempted to cross Interstate 95. The pedestrian was then struck by an unknown vehicle, possibly a tractor trailer.
