Requirements to Open a Commercial Driving School

An individual interested in opening a Commercial Driving School must submit:

  1. An Original Commercial Driving School Application for licensure. (Note: Please type in the form, print it then, save it.)


  2. A Certified fictitious name or Certificate of Incorporation filed with the Secretary of State:

  3. a. Certified fictitious name must have been issued within the last five years.
    b. Certificate of Incorporation must have been filed within the last year.

  4. A copy of the contract to be issued between the school and student.


  5. A copy of the training curriculum (for classroom instruction).


  6. A copy of a valid certificate of insurance for each vehicle used for instruction.


  7. An original FDLE background report for each owner, partner, director, officer, or principle stockholder.
    a. Must be issued with in the last 12 months.

  8. Documentation for each certified instructor. (See Requirements to Become a Certified Instructor)


  9. A certificate of occupancy or lease agreement for the office; the office location can not be attached to a residence.


  10. A signed and witnessed agreement with the department. (Note: Please type in the form, print it then, save it.)


  11. A $250 payment; includes $200 license fee plus $ 50 application fee.

Requirements to Become a Certified Instructor  

To become a certified commercial driving school instructor you must have no suspensions, revocations, cancellations, or disqualification on your record 3 years prior to the date of the application and you must submit:

  1. A completed commercial driving school instructor application (Note: Please type in the form, print it then, save it.)

  2. A Certificate of Completion for: 32-hour Driving Instructor Training Course (DITC), 8-hour DITC Refresher, or Driver Performance Analysis System (DPAS). The DPAS is a test given to potential instructors who have not taken the 32-hour DITC. If you submit the DPAS scores, you must enroll and complete the next scheduled 32-hour DITC.   3. An original FDLE report:


    • a. Must be issued with in the last 12 months.
      b. A candidate who has not resided in the state for 12 months should also include a report from the previous state of residence.

  4. A letter of employment from school owner.


  5. A signed and witnessed agreement with the department.
