The Florida Highway Patrol has received information that forms are being mailed out to various people, allegedly from the Internal Revenue Service (IRS). The packet requests that the forms be filled out and mailed back to the IRS. FHP investigators have determined that the forms are not from the IRS and the mailing address indicated is not to any IRS location.

The forms are apparently being used to obtain personal identification information so thieves can then commit identity theft and steal bank accounts. The form numbers are W-9095 and W-8888.
