Last November 20, 2009, we posted Technical Advisory R09-08 regarding form numbers we issued in error. At that time, we issued form number 82191 to replace the wrong form number 86066. We allowed you time to use the forms you had on hand before requiring you to use the new form 82191. That transition period is over now. If you are still using form number 86066, please discontinue doing so immediately. This form is obsolete; we will only accept form HSMV 82191.
You may continue to order pads of form HSMV 82191 from DHSMV Supply while they last. Once we deplete our stock, however, we will no longer offer hard copy of this form. At any time, you may obtain this form from our PartnerNet website.
We understand that some tax collector offices do not know how to access form 82191. We do not provide this form on our Internet site because it is not a public form. However, we do provide this form on our PartnerNet site.
