Effective immediately, any form HSMV 83363, Application for License Plate/Decal Refund, with a revision date prior to 10/09 is obsolete. Please destroy all obsolete forms you may have in stock and make sure that customers are given the form with the 10/09 revision date. Applications submitted with a revision date prior to 10/09 will be rejected.
