DHSMV Traffic Crash Report Home
Effective 12:00 AM, January 1, 2011, law enforcement agencies must
submit all NEW Long Form Crash Reports to the Department utilizing the
revised Crash Report (HSMV 90010 S). Updates or corrections to Long
From Crash Reports previously submitted, before January 1, 2011, should
use the same crash report format as the original submission.
There are three ways for completing and submitting Long Form Crash Report HSMV 90010 S:
- Officers
will handwrite (or type) crash reports and they will be mailed to the
Department. Refer to the Printed/Mailed section below.
- Officers
will enter crash data using a forms application product. The reports
will then be printed and mailed to the Department. Refer to the
Printed/Mailed section below.
- Officers
will enter crash data using a forms application product. Those reports
will be sent electronically adhering to the Department’s XML
standard. Refer to the Electronic Submission section below.
